Signature

I will be on maternity leave until September 5th 2017, so until then please follow these instructions if you need my signature in any of these forms:
- Mrs. Beth Bucher, our MS Program Cooridnator, has kindly agreed to sign "Course Enrollment Forms" for me. She has walk-in hours from 2pm to 4pm, Monday through Thursday in DL 205. Please go see her if you need my signature on a "Course Enrollment Form";
- Please email me "OPT" and "Reduced Course Load" forms*, I will sign them and email them back to you within a couple of days.

 

 

After September 5th, if you need my signature for:

Course Enrollment Form

You can either:

  1. Schedule an appointment with me and bring the form already filled out to the meeting;
  2. Send me an email asking me permission to enroll in the course. In the email, you have to specify the course #, class #, term, instructor and # of credits. Once I reply granting you the permission, all you need to do is to forward  the email** to the Graduate School. If the instructor’s signature is also needed, you have to email and receive permission to enroll from both of us.

Please schedule an appointment with me and bring the form already filled out to the meeting.

“OPT” and “Reduced Course Load” application

You can either

  1. Schedule an appointment with me and bring the form already filled out to the meeting;
  2. Scan* the form (filled out) and send it to me via email. I will sign it and email it back to you.

 


*   There is a scanner for students to use in Caldwell 260.  It is next to the three printers on the right.

**   The Graduate School email that you have to use is grad-schoolregistrationservices@osu.edu
